TERMS AND CONDITIONS

1. General

a) practice CeeYu is a therapeutic coaching practice aimed at guiding individuals, partner relationships and employees. Practice CeeYu was founded by Maarten van Zwieten and is located at Eendrachtstraat 16 in Terschuur. The practice is registered with the Chamber of Commerce under number 27293043.

b) This website serves to provide general information. By accessing this website and the material and information contained therein, you declare that you have read, understand, and agree to be bound by the following terms and conditions. During the intake interview, reference is made to these general terms and conditions.


2. Definitions

a. Contractor; Practice CeeYu

b. Client; The counterparty of the contractor, namely the client or employer of the client


3. Work field Practice CeeYu

a) The Contractor focuses on treating and coaching individuals, partners (couples) and employees.

b) When treating a minor client (between 14 and 18 years old), the parent (or parents) will be kept informed of the progress of the process. Consultation with parents takes place prior to the process. In consultation with the client, the parent (or parents) is present at the intake interview.


4. Limits of assistance

a) It is the responsibility of the client to inform his/her general practitioner and/or therapist of his/her trajectory at practice CeeYu.

b) It is the responsibility of the client to paint a sincere picture of his/her psychosocial problems and physical condition during intake and coaching conversations. Also with regard to medication and other therapies.


5. Confidentiality

a) The coach/therapist undertakes to maintain the confidentiality of everything discussed during or in the context of the coaching and treatment sessions.

b) This confidentiality can be lifted in mutual consultation between coach and client, in case of referral or informing other care providers or parents.

c) In the event of imminent danger to both the client and society, Practice CeeYu reserves the right to break the confidentiality and to inform the competent and appropriate authorities.

d) When handling the personal (name and address) data of the client, CeeYu practice adheres to the privacy rules of the General Data Protection Regulation (GDPR).

e) The coach/therapist reserves the right to make audio recordings of a session, solely for the purpose of possible (anonymous) discussion during peer peer review or supervision.


6. Terms of Payment

a) An invoice will be sent to the client after each intake or coaching conversation. Payment must be made within a period of 14 days from the invoice date. Payment can be made, if desired, via a tikkie request.

b) In the event of late payment of the invoice, reminder or failure to pay, Practice CeeYu reserves the right to terminate the coaching or treatment process with immediate effect.

c) In the event of any objections regarding the invoice, the client must make this known to the contractor in writing within two weeks of receiving the invoice.


7. Making and canceling appointments.

a) Appointments for intake or coaching conversations are made in good consultation and always recorded or confirmed in writing.

b) If the client cancels the appointment made by her/him within 24 hours before the relevant appointment, practice CeeYu will charge the full costs and the client is obliged to pay these. Appointments canceled or rescheduled before this time will not be charged.

c) If the client does not appear for a scheduled meeting, the full costs for the meeting will be charged and the client is obliged to pay these.


8. Liability

a) The coaching and guidance by practice CeeYu is aimed at self-insight, self-reflection and personal growth, without guaranteeing the result.

b) Practice CeeYu is never liable for direct damage or indirect damage, emotional damage or damage resulting from or in connection with the services offered, unless there is intent or gross negligence on the part of Practice CeeYu.

c) The coach/therapist of practice CeeYu can never be held responsible for what happens outside the sessions.

d) In the event of physical and psychological complaints, the contractor expressly recommends that you first consult a doctor or psychologist.

e) The client is at all times responsible for choices made, his own behavior and the consequences thereof, both during the period of the coaching or treatment sessions and the period afterwards.
